 ウィリアムス症候群は、7番染色体長腕の微細欠失症候群(7q11.23)で、発生頻度の稀な神経発達障害である。心臓疾患、特徴のある顔貌、聴覚過敏知的障害視空間認知の障害、高い社交性などの特徴を伴う。言語能力の優位性と視空間認知能力の障害という能力の不均衡があると言われてきたが、近年この考えは言語能力と認知能力の乖離を誇張しがちであるとも批判されている。視空間認知の障害として、脳の背側経路の障害が指摘されている。また、社交性の高さについては、扁桃体の機能不全が指摘されている。

 大動脈弁上狭窄、短い眼瞼裂や低い鼻根などの特徴的な顔貌(図1)、聴覚過敏、低身長などの身体的特徴を伴い、乳児期に高カルシウム血症を示す患者もいる[2]

神経発達

 平均知能指数が55程度で、軽度から中度の知的障害を持つ人が多い[3]。ウィリアムス症候群を持つ人の特性の中で特筆すべきものとして、言語能力の優位性と視空間認知能力の障害という能力の不均衡があると言われてきた[4][5]。そのことにより、言語能力のモジュール説(言語が他の脳領域から独立して機能していることを主張する立場)を支持する症例として研究者らに取り上げられてきたことがあった[6]。しかし、近年この考えはウィリアムス症候群における言語能力と認知能力の乖離を誇張しがちであるとし批判され[7]、その批判を支持する研究が多い[8][9]

 視空間認知では、積み木の模様構成や描画でかなりの困難を示す。ウィリアムス症候群を持つ子どもの描画能力の例として、図2がある[10]。その一方で、顔の認識能力は他の能力と比べて高く、複数の顔写真の中からターゲットとなる顔と(角度や照明の状況が異なっている)同じ顔を選択させるベントン顔認識テスト(Benton Test of Facial Recognition)で、生活年齢と同等かそれに近いレベルの成績を示すことが報告されている[11][12]

 言語・社会能力では、他の能力に比べて高い語い能力を有する。しかし、文法能力は精神年齢と同程度で、語用能力(文脈に応じて適切に言葉を理解・使用する能力)にも困難を抱えるという報告がある[13][14]。また、初対面の人にも躊躇なく接することや人とたくさん話すという高い社交性を持つ[15][16]が、その高すぎる社交性によりトラブルになるまたは巻き込まれるということもある[17]

診断

 7番染色体長腕の微細欠失症候群(7q11.23)で、エラスチン遺伝子(elastin; ELN)を含む約1.6Mbの領域の約20の遺伝子が欠失していると言われ[18]、診断は臨床症状の診察を行い、この領域の欠失を判定するFISH法により確定される。

疫学

 発生頻度は、従来は2万人に1人と言われていたが、最近では7500人に1人の割合という報告がある[19]

神経基盤

 神経基盤としては、視空間認知の障害として、脳の背側経路の障害が指摘されている[20][21]。Meyer-Lindenbergら[22]は、fMRIの知見などから、腹側経路ではそれほど問題がないものの、背側経路で低活性が見られ、さらに頭頂間溝の構造異常が見られることからこの部分から背側経路への情報入力に問題がある可能性を指摘している。ウィリアムス症候群における頭頂間溝の構造異常が視空間認知障害と関わるという指摘は、他の研究でもなされている[23]

 また、社交性や不安については、扁桃体の関与が指摘されている。ある研究では、人画像に対して扁桃体の活性が低く、物体画像に対しては扁桃体の活性が強いという結果が見られた[24]。このことは、人に対する親密性と特定の物体(例:注射)に対する過度の不安を示すというウィリアムス症候群を持つ人の行動パターンをよく表している。さらに、扁桃体の大きさがウィリアムス症候群を持つ人では定型発達者よりも大きく、その大きさと人に対する親密性が関連するという報告がある[25]

治療

 聴覚過敏などに伴う不安のコントロール、高い社交性(例:人に近づきすぎる)がトラブルを生まないような配慮が必要である。また、一見して分かる、多弁や高い社交性という特徴だけにとらわれず、ウィリアムス症候群を持つ人の得手・不得手を見極めた対応が重要である。(編集コメント:薬物治療などがございましたらご指摘下さい)。
