 プリン受容体は、アデノシンならびにATPなどヌクレオチド類をリガンドとする一群の細胞表面受容体である。アデノシンをリガンドとするP1受容体ファミリー、ATPをリガンドとするP2受容体ファミリーに分類される。P1受容体は、Gタンパク質共役型受容体(GPCR)で、A1、A2A、A2BおよびA3に分類される。P2受容体は、さらにリガンド依存性イオンチャネル型受容体であるP2XおよびGタンパク質共役型受容体であるP2Y受容体の2種類に分類される。それぞれの受容体を活性化するリガンドの種類や濃度、また発現組織分布や発現細胞種などが異なり、生体機能におけるサブタイプ固有の役割が徐々に明らかになってきている。

プリン受容体とは

 プリン受容体は、P1およびP2受容体ファミリーに分類され、P1はアデノシン受容体(adenosine receptor)、P2はATP受容体(ATP receptor)ともいわれる[1] [2]

 P1受容体は、内因性リガンドがアデノシンであるGタンパク質共役型受容体(GPCR)で、A1、A2A、A2BおよびA3に分類される。

 P2受容体は、細胞外のプリンヌクレオチド(ATP、ADP)、ピリミジンヌクレオチド(UTP、UDP)、糖ヌクレオチドなどを内因性リガンドとし、さらにP2XおよびP2Y受容体の2種類に分類される。P2X受容体は、細胞膜を2回貫通するサブユニット(7種類:P2X1~P2X7)3分子がホモあるいはヘテロ三量体を形成し、一つのチャネル(P2X受容体)となる。P2X受容体は、Na+、Ca2+およびK+いずれも通す非選択的陽イオンチャネルである。P2X7以外のP2X受容体は、1~10 μM程度の細胞外ATPにより活性化される(P2X7受容体だけは活性化本体がATP4-と考えられているため、その活性化に非常に高濃度のATP(0.1~1 mM)が必要である)。P2X1とP2X3受容体はATP刺激により急速に不活性化し、繰り返し刺激により著明な脱感作を示すが、P2X2、P2X4、P2X5、P2X7受容体はそれらが軽度である。P2Y受容体は、7回膜貫通型のGPCRで、8種類(P2Y1、P2Y2、P2Y4、P2Y6、P2Y11〜P2Y14)に分類される。なお、本項におけるP1およびP2受容体の表記は、IUPHAR 国際薬理学連合でのデータベース掲載名に従った。

 このように細分化されたプリン受容体ではあるが、それぞれの受容体を活性化するリガンドの種類や濃度、また発現組織分布や発現細胞種などが異なり、生体機能におけるサブタイプ固有の役割が徐々に明らかになってきている[1] [2]

P1受容体

A1受容体

(A1 receptor、遺伝子名:ADORA1(ヒト)、Adora1(ラット、マウス))

 Gi/Go タンパク質共役型受容体である。カフェインの作用標的としても知られている。脳や脊髄などの中枢神経系組織に加え、心臓や腎臓、肺といった末梢組織を含む全身の様々な組織に発現している[3] [4]。A1受容体はP2Y1受容体とヘテロマーを形成することも知られている[5]。これまでに数十種類の選択的作動薬拮抗薬が合成され、その機能解明も進んでいる。例えば、脳において興奮性シナプス伝達を抑制的に制御していることが知られており[6]、A1受容体欠損マウスの海馬においては、シナプス伝達の異常興奮が観察される[7]。また、心筋の収縮力や心拍数の制御に加え、血管拡張作用、体温調節機構にも関与していると考えられている[7] [8]。A1受容体欠損マウスは、てんかん症状や痛覚過敏症状、不安関連行動を呈するほか、インスリン分泌の亢進や低酸素性障害や虚血ストレスに対する抵抗性の低下が観察される[7] [9]

A2A受容体

(A2A receptor、遺伝子名:ADORA2A(ヒト)、Adora2a(ラット、マウス))

 Gs共役型GPCRで、受容体刺激によりアデニル酸シクラーゼが活性化され、cAMPの産生が亢進する。A1受容体[10]や他の神経伝達物質受容体(D2受容体[11], D3受容体[12], 代謝型グルタミン酸受容体mGluR5[13], CB1受容体[14])とヘテロマーを形成することがある。A1受容体と同様に、カフェインが拮抗薬として作用する。また、A2A受容体に対する選択的作用薬としてCGS21680やDPMA、拮抗薬としてistradefyllineやSCH-58261がある[15]。A2A受容体は、生体内に幅広く分布し、線条体や海馬、冠血管、肺、血小板、腎臓などに発現する[16] [17] [18]。生理的役割としては、血管拡張、睡眠、神経活動制御があり、A2A受容体欠損マウスにおいて不安行動及び攻撃性増加や痛覚鈍麻、心拍数増加、血圧上昇、血小板凝集が見られる[19] [20]。最近、ミクログリア細胞にA2A受容体が発現し、突起の退縮に関与していることも報告された[21]。現在、A2A受容体作動薬Regadenoson(レキスキャン®、アステラス製薬/CVセラピューティクス)が心筋血流イメージングの薬物負荷剤として、A2A受容体拮抗薬イストラデフィリン(ノウリアスト®、協和発酵キリン)がパーキンソン病治療薬として用いられている。

A2B受容体

(A2B receptor、遺伝子名:ADORA2B(ヒト)、Adora2b(ラット、マウス))

 A2A受容体と同様にGs共役型GPCRであるが、アデノシンへの親和性が低く、アデノシンの他にnetrin-1の受容体としても機能する[22]。A2B受容体選択的作動薬としてBAY 60-6583やNECA、拮抗薬としてMRS-1754やMRE-2029-F20、CVT6883、PSB-1115などがある[15]。A2B受容体は、中枢神経系を含め、全身に広く発現しており[23]、特に脳や腸、腎臓、肺に多く、心臓や大動脈にもわずかに発現が見られるが、肝臓ではほとんど見られない[24]。また、肥満細胞や繊維芽細胞にも発現する[23] [25]。A2B受容体欠損マウスにおいて、リポ多糖(LPS)による炎症及びサイトカイン産生の抑制やリンパ球の血管付着の増加[24]、肥満細胞の活性化及びIgE誘発アナフィラキシー亢進[25]、心筋虚血プレコンデショニングによる心臓保護作用の抑制が報告されている[26]

A3受容体

(A3 receptor、遺伝子名:ADORA3(ヒト)、Adora3(ラット、マウス))

 薬理学的機能同定前にクローニングされた唯一のアデノシン受容体サブタイプで、Gi、GoあるいはGqタンパク質と共役し細胞内にシグナルを伝える[6]。肺や腎臓、心臓、脳、脾臓、肝臓など全身の様々な組織に発現しているが、その発現レベルは動物種間で大きく異なっている[4] [27]。例えば、ラットでは睾丸や肥満細胞で発現量が高いのに対して、人では肺や肝臓でその発現が高く、脳や大動脈での発現量は低い[3]。A3受容体欠損マウスにおいては、野生型マウスと比較して、眼圧の低下[28]や局所炎症反応の減少[29]など、いくつかの表現型の違いが観察される。

P2X受容体

P2X1受容体

(P2X1 receptor、遺伝子名:P2RX1(ヒト)、P2rx1(ラット、マウス))

 P2X1受容体は血小板やマスト細胞、リンパ球といった血液細胞に多く発現する。血小板においては、トロンビン受容体を介した血小板の凝集に関与することが分かっており、P2X1受容体欠損マウスにおいて血栓形成の抑制および出血時間の延長が見られる[30] [31]。また、好中球にも発現し、ATP誘発性の遊走を引き起こすことで免疫反応に関与する[32]。一方、平滑筋にも多く発現し、収縮を引き起こすことが分かっており、腎血管の自動調節能や男性不妊症などへの関与が報告されている[33] [34]。中枢神経系では、アストロサイトに発現している[35]が、その機能や役割は明確にはなっていない。

P2X2受容体

(P2X2 receptor、遺伝子名:P2RX2(ヒト)、P2rx2ラット、マウス))

 P2X受容体の中で最も広範に発現する受容体であり、中枢(嗅球大脳皮質、基底核、間脳、中脳、小脳、延髄および脊髄後角)および末梢神経系(感覚神経節および自律神経節)に特に高発現している[1] [36]。一方で、P2X2受容体欠損マウスでは、低酸素に対する換気応答への関与が示されているものの[37]、著明な神経活動の異常は認められないため、その生理的役割には不明な点が多い。末梢では、網膜や蝸牛、味蕾などに発現が見られる[1] [38]。蝸牛においては、蝸牛内電位の調節に関与することが分かっており、騒音による発現上昇や難聴への関与が報告されている[39] [40]。また、P2X2受容体は、P2X3受容体とヘテロ三量体(P2X2/3ヘテロマー受容体)を形成することも知られており[41]、痛み信号の発生や膀胱反射機能に関与している[42] [43]。最近では、前頭葉皮質のP2X2受容体を刺激することでうつ病様行動が抑制されることがマウスの実験で示されている[44]

P2X3受容体

(P2X3 receptor、遺伝子名:P2RX3(ヒト)、P2rx3ラット、マウス))

 一次求心性感覚神経の主にC線維に高発現する[45]。侵害刺激が加わると活動電位を引き起こし、侵害受容性疼痛や神経障害性疼痛に関与する。また、P2X2/3ヘテロ受容体としても存在し、線維において機械的アロディニアの発生に関与する[42]。P2X3受容体欠損マウスでは疼痛行動の抑制[46] [47]、さらにP2X3受容体選択的拮抗薬(A317491)による鎮痛効果が報告されている[48]。第3世代ビスホスホネート製剤であるミノドロン酸は、P2X2/3受容体阻害作用と鎮痛作用を示す[49]。生理的機能としては、蠕動反射や膀胱容量反射への関与が明らかとなっており[46]、過敏性腸症候群や尿路機能障害への関与も報告されている[1] [2]

P2X4受容体

(P2X4 receptor、遺伝子名:P2RX4(ヒト)、P2rx4ラット、マウス))

 P2X4受容体は、他のP2X受容体よりカルシウム透過性が高いことが特徴である[50]。また、腸管糞線虫症の駆虫薬イベルメクチンによりアロステリックにATPの作用が増強される[51]。P2X4受容体は、ヒトにおいて脳や脊髄、心臓、肺、肝臓、腎臓など幅広い器官に発現している[52]ゼブラフィッシュ由来P2X4受容体の閉状態に相当するアポ型や開状態に相当するATP結合型のX線結晶構造も明らかとなっている[53] [54]。細胞内での分布は、リソソームに局在する特徴を持つが、リソソーム内腔側のP2X4受容体にある複数の糖鎖のためタンパク分解を免れている[55]。脊髄のミクログリア細胞のP2X4受容体が神経障害性疼痛に重要であること[56]や、マクロファージのP2X4受容体が炎症性疼痛に関与することが報告されている[57]。また、血管内皮細胞に発現するP2X4受容体は血管拡張反応や血流変化により誘導される血管のリモデリングにも関与する[58]パロキセチンなどの抗うつ薬がP2X4受容体に対して阻害作用を有することも認められている[59]

P2X5受容体

(P2X5 receptor、遺伝子名:P2RX5(ヒト)、P2rx5ラット、マウス))

 ニワトリおよびカエル由来のP2X5受容体はATPに応答するものの、哺乳類やゼブラフィッシュP2X5受容体は非常に小さい応答しか示さない[36]。また、ヒトP2X5受容体はエクソン10が欠損しているため、機能的な受容体にはならない。一方で、P2X5受容体は、ASIC3と分子複合体を形成し、pH感受性を増加させ、筋肉虚血による低pHやATPの感知に関与していることが報告されている[60]。神経系における役割は不明である。

P2X6受容体

(P2X6 receptor、遺伝子名:P2RX6(ヒト)、P2rx6ラット、マウス))

 P2X6受容体はATPに対して非常に微弱な応答しか誘発せず、ホモ三量体を形成できないという報告もある[36]。中枢や末梢神経系におけるP2X6受容体の発現分布がP2X2やP2X4受容体と類似していることから、P2X6はこれらのP2X受容体とヘテロ三量体受容体として機能している可能性が考えられている。しかし、P2X6受容体の機能や役割は明らかになっていない。

P2X7受容体

(P2X7 receptor、遺伝子名:P2RX7(ヒト)、P2rx7ラット、マウス))

 以前はP2Zとも呼ばれていた。他のP2X受容体に比べて細胞内のC末端が非常に長いのが特徴的であり、他のタンパク質と物理的に相互作用することが報告されている[61]。作動薬としてBzATP、拮抗薬としてPPADSやBBGが知れているが、最近では、多くの選択的拮抗薬(A-317491、A-438079、AZ11645373など)が開発されている[62]。P2X7受容体は、マクロファージやミクログリア、単球、肥満細胞、リンパ球および表皮ランゲルハンス細胞など主に免疫系の細胞で多く発現している[63]。P2X7受容体はサイトカイン産生やアポトーシスを制御することが知られ、アルツハイマー病やパーキンソン病、多発性硬化症、骨粗しょう症、神経障害性疼痛など様々な病態に関与する[63] [64] [65] [66]。また、P2X7受容体をコードする遺伝子のコーディング配列内の変動が、マウスとヒトの両方で慢性疼痛の感受性に影響を及ぼすことも報告されている[67]。P2X7受容体機能は複数の研究グループにより樹立されたP2X7遺伝子欠損マウスで解析されているが、P2X7受容体機能が完全に欠失していないものもあり、表現型の解釈には注意が必要である[36]


P2Y受容体

P2Y1受容体

(P2Y1 receptor、遺伝子名:P2RY1(ヒト)、P2ry1(ラット、マウス))

 P2Y1受容体は主にADPをリガンドとするGq/11共役型の受容体である。生体内で広範に発現し、主に上皮細胞、内皮細胞、血小板、免疫細胞や破骨細胞に発現する。P2Y1受容体欠損マウスを用いた解析から、出血時間の増加やADP誘発の血小板の凝集および血栓形成に異常が見られることから血小板の機能に重要な役割を果たしていると考えられる[68] [69]。また、中枢神経系において視床下部におけるP2Y1受容体の発現が食物摂取に関与することが示唆されている[70]。P2Y1受容体はA1受容体とヘテロ受容体を形成し[5]、海馬ニューロンからのグルタミン酸放出を抑制性に制御している[71]。さらに、アストロサイトに発現するP2Y1受容体の活性化により、酸化ストレスによるアストロサイトのダメージが抑制され[72]、脳虚血/再灌流時における脳障害も抑制する[73]

P2Y2受容体

(P2Y2 receptor、遺伝子名:P2RY2(ヒト)、P2ry2(ラット、マウス))

 ATPおよびUTPを内因性リガンドとするGq/G11共役型の受容体である。生体内で広範に発現が確認されており、主に免疫細胞、内皮細胞、上皮細胞、腎臓、骨芽細胞などで発現している。P2Y2受容体欠損マウスの解析から、神経細胞の軸索伸長や分化への関与[74]や、気道上皮細胞におけるCl-の放出への関与が示唆されている[75]。炎症時における神経保護作用に関する役割にも注目が集まっている[76]。マクロファージでのP2Y2受容体は、アポトーシス細胞が放出するATPで刺激され、貪食によるクリアランスに関与する[77] [78]。現在、P2Y2受容体作動薬デヌホソル(Inspire Pharmaceuticals)が嚢胞性線維症の吸入治療薬として開発されており(フェーズⅢ、USA)[79] [80]、P2Y2受容体の遺伝子多型の一つが健常人に比べ嚢胞性線維症の患者において高頻度でみられることから、嚢胞性線維症の発症および治療への関与が考えられる[81]。さらに、ドライアイ治療薬としてP2Y2受容体作動薬ジクアホソルナトリウム(ジクアス®点眼液3%, 参天製薬)がある。

P2Y4受容体

(P2Y4 receptor、遺伝子名:P2RY4(ヒト)、P2ry4(ラット、マウス))

 主にUTPをリガンドとするGq/11共役型の受容体である。腸管に豊富に発現しており、その他にも精巣、下垂体および脳などで発現が報告されている。P2Y4受容体ノックアウトマウスでは、空腸上皮からのUTP誘発のCl-の放出の消失および大腸粘膜からのUTP誘発のK+の放出が減少する[82] [83]。さらに、心臓の内皮細胞での発現が確認され、P2Y4受容体ノックアウトマウスにおいて心臓の発達障害が見られ[84]、加えて運動能力の低下が認められる[85]。中枢神経系においては、アストロサイトで発現しており、シナプス形成や再構築に重要なthrombospondin-1(TSP-1)の発現誘導に関与することが報告されている[86]

P2Y6受容体

(P2Y6 receptor、遺伝子名:P2RY6(ヒト)、P2ry6(ラット、マウス))

 主にGq/11と共役しIP3受容体を介した細胞内カルシウム濃度の上昇を引き起こす[1]。また、心筋細胞ではG12/13を活性化する[87]。内因性リガンドはUDPであり、比較的高濃度のUTPやADPも部分的作動活性を示す。生体内では幅広い組織で発現が確認されている[1]。P2Y6受容体ノックアウトマウスでは、定常状態での著明な表現型はないものの、骨組成の変化やUDP刺激によるマクロファージからのサイトカイン産生や血管平滑筋収縮能が欠失している[88]。LPSや腫瘍壊死因子α(TNFα)刺激後の血管内皮細胞、チオグリコール酸誘導腹腔マクロファージ、脳虚血再灌流後のミクログリアで発現が増加するなど、炎症時に発現が変化する[89]。また、ミクログリアのP2Y6受容体は、死細胞の貪食応答に関与する[90]

P2Y11受容体

(P2Y11 receptor、遺伝子名:P2RY11(ヒト))

 主にGq/11と共役しIP3受容体を介した細胞内カルシウム濃度の上昇を引き起こす。内因性リガンドの中で、ATPへの選択性が高く、UTPやUDPには応答しない。高濃度のリガンド存在時はアデニル酸シクラーゼの活性化を引き起こすという報告もある。マウス、ラットのゲノムには存在しておらずカニクイザルのP2Y11受容体はヒト受容体と70%の相同性であるがATPよりもADPによって強く活性化される。生理学的な役割はいまだ明らかにされておらず、顆粒球の分化過程、樹状細胞の成熟や移動能についての報告がある。ヒト組織での発現は脾臓、肝臓、腸管、脳、脳下垂体で報告されている[1][91]

P2Y12受容体

(P2Y12 receptor、遺伝子名:P2RY12(ヒト)、P2ry12(ラット、マウス))

 Giタンパク質と共役しアデニル酸シクラーゼを抑制する。Gβγの作用でPI3キナーゼを活性化するという報告もある。ADPを内因性リガンドとし、ATPやその類似化合物は拮抗薬として作用すると報告されている。主に巨核球・血小板でその発現が見られGαi2を介したシグナルによってP2Y1受容体とともに血小板凝集作用を示す[91]。中枢神経系ではミクログリアで高発現し、細胞の移動・突起伸展に関わっている[92] [93]。阻害薬の抗血小板作用が注目され、プロドラッグで非可逆的なP2Y12受容体阻害薬であるクロピドグレル(プラビックス®、サノフィ・アベンティス)、プラスグレル(エフィエント®、第一三共)が医薬品として認可を受けおり、可逆的阻害薬のチカグレロル(ブリリンタ、アストラゼネカ)も開発中である。

P2Y13受容体

(P2Y13 receptor、遺伝子名:P2RY13(ヒト)、P2ry13(ラット、マウス))

 P2Y12受容体との配列相同性が高く、Gi共役型であることやADPへの応答性などの点で類似している。脾臓、肝臓、膵臓、脳、心臓[94]、脊髄、後根神経節[95]、単球[96]等、幅広い組織で発現している。近年、HDLコレステロールの逆輸送や胆汁酸分泌への関与が示唆されている[97]。P2Y13受容体欠損マウスを用いた検討により、細胞外でのATP代謝や機械刺激に対する骨形成応答への関与が示されている[98]

P2Y14受容体

(P2Y14 receptor、遺伝子名:P2RY14(ヒト)、P2ry14(ラット、マウス))

 Gi/o共役型受容体である。他のP2Y受容体と異なり、ウリジンヌクレオチドやアデニンヌクレオチドには応答せず、UDPグルコースなどの糖ヌクレオチドを内因性リガンドとする[99]。生体内においては、脾臓、胸腺、腸管、脳、肺、心臓、骨格筋等、幅広い組織での発現が確認されている[99] [100]。明確な役割については解明されていないが、ケモタキシスや肥満細胞の脱顆粒、神経免疫調節などへの関与が示唆されている[101] [102]
