 通常では痛みを引き起こさないような非侵害刺激(接触や軽度の圧迫、非侵害的な温冷刺激など)で痛みを生じてしまう感覚異常のこと。国際疼痛学会(IASP)では、「Pain due to a stimulus that does not normally provoke pain.」と定義されている。末期がん、糖尿病性ニューロパチー帯状疱疹後、脊髄損傷などで発症する神経障害性疼痛の代表的症状の一つである。

 アロディニアのメカニズムは未だ不明であるが、一次求心性感覚神経/C線維の閾値が低下した場合やAβ線維痛覚伝導路を経由して痛みを伝える可能性が考えられている[1] [2]。また、脊髄後角ニューロンの興奮性シナプス伝達の増加、抑制性シナプス伝達の低下が関与していることも報告されている[2] [3]。さらに最近の研究では、脊髄のグリア細胞ミクログリア[4] [5] [6]やアストロサイト[7] [8])の活動性の増加が脊髄後角ニューロンの過剰興奮を引き起こし[9]、アロディニアの発症維持に重要な役割を担っていることも示唆されている[10] [11]
