 補足運動野(supplementary motor area, SMA)とは大脳皮質前頭葉のうちBrodmann分類の6野内側部を占める皮質運動領野である。Penfield, Welchによって初めてその存在が報告され、それまでに知られていた一次運動野に対して、もう一つの補足的な皮質運動領野であるという意味を込めて命名された。しかしその後の研究によって補足運動野は運動制御において一次運動野とは異なる固有の役割(例、自発的な運動の開始、異なる複数の運動を特定の順序に従って実行する、両手の協調動作など)を果たしていることが明らかにされている。なお、当初補足運動野は6野内側部全体を占めていると考えられていたが、現在では補足運動野は6野内側部後方を占める一方、6野内側部前方部は前補足運動野として区別される(図1)。このため、補足運動野は前補足運動野との区別を強調する意図でcaudal SMA, SMA properなどと呼ばれることもある。

歴史的背景

 補足運動野は20世紀中盤にカナダの脳外科医Wilder Penfield及びKeasley Welchによって発見・命名された[1]。大脳皮質に於いては中心溝に接する前頭皮質(中心前回)に運動を支配する領域(一次運動野)が存在する事が古くから知られていたが、Penfieldらは中心前回の更に前方、Brodmannの6野内側部に電気刺激によって運動が誘発される領域があることを見出した。この領域における誘発運動には体部位再現が存在する。つまり刺激部位によって前方から後方にかけて刺激側とは反対側の顔、上肢、体幹、下肢の順に異なる体部位の運動が誘発され、かつこの体部位再現は一次運動野のもの(外側から内側にかけて顔、上肢、体幹、下肢)とは位置的にも別個のものである。更に、サルを用いた実験では補足運動野と一次運動野の間の連絡線維を切除した後でも補足運動野の電気刺激によって運動を惹起できることから、補足運動野は一次運動野とは独立した皮質運動領野として確立された。
 後の研究によって、補足運動野の位置する6野内側部には前後各一つずつの運動関連領野が存在することが判明し、そのうち従来から知られていた補足運動野の性質(体部位再現の存在、電気刺激による運動の誘発、脊髄への投射経路の存在など)は6野内側部後方の領域に当てはまる事が判明したため、現在では6野内側前方部を前補足運動野、後方を本来の意味での補足運動野として区別する。以下、本項目ではこの新しい定義による補足運動野を取り扱う。

解剖・生理学的所見

 補足運動野はBrodmann6野内側部後方に位置する[2][3]。補足運動野は運動系中枢との密な線維連絡を持ち、脳幹運動神経核や脊髄運動細胞へ直接投射する[4][5]ほか、一次運動野、背側及び腹側運動前野帯状皮質運動野とも双方向性に結合する[6]。頭頂葉との関係では、二次体性感覚野やBrodmannの5野からの入力を受ける[6]。一方で前頭前野前頭眼窩野とは直接の線維連絡を持たない[6]。また補足運動野は視床VLo核を介して大脳基底核からの入力を受け取る一方、小脳核からの入力は乏しい[7][8][9][10]。対照的に一次運動野運動前野は小脳からの入力が優勢である[7][10][11]
 前述のように補足運動野には電気刺激による誘発運動や体性感覚応答の受容野によって定義される体部位再現があり、前方より顔、上肢、体幹、下肢の領域が認められる[12][13][14]。一方で視覚刺激に対する応答性は乏しく、この点で前補足運動野とは区別される。

機能

 補足運動野の機能に関しては、脳血管障害などに伴う破壊症状や動物・人間における電気生理学的研究、脳機能イメージング等から様々な仮説が提唱されている。ここではそのうち重要なものについて触れる。

随意的な運動の開始及び抑制

 一次運動野と異なり補足運動野の損傷は軽微な麻痺しか起こさず、一見すると運動の制御に従属的な役割しか果たしていないように見える。しかし補足運動野の損傷は自発的な発語や運動の開始が著しく困難になる無動性無言症(akinetic mutism)と呼ばれる特徴的な症状を惹き起こす。一方でこうした患者でも本を渡して「声を出して読みなさい」と指示されると問題なく読むことが出来、験者が行う動作を真似する限りはなんら障害を示さない。つまり運動の遂行自体に障害はなく、外部から何をいつ為すべきか指示を与えられると運動を遂行できるが、自発的に運動を開始できないのである。動物実験からも同様の所見が得られている[15]。こうした所見からは補足運動野は自発的な運動の開始に寄与している事が窺われ、実際、ヒトでは自発運動の開始に先行して補足運動野領域から運動準備電位(Bereitschaftspotential)が記録される[16]。又、サルの補足運動野のニューロン活動を記録した研究によっても、補足運動野のニューロンは動物が外部からの指示に拠らずに運動を実行する際に活動する傾向があることが指摘されている[17][18]
 補足運動野の損傷は自発的な運動の開始に困難をもたらす一方で、意図しない運動の出現をもたらす。補足運動野の損傷によって生じる非意図的な運動の代表例として挙げられる”他人の手症候群(alien-hand syndrome)”では、患者の手が本人の意思とは無関係にまるで他人の手であるかのように一定のまとまった動作(例、健側の手が片付けた物を患側の手が勝手に取り出すなど)を行う。その他にも道具の強制使用、強制把握などの非意図的な運動が生じる。我々の脳は五感を通して外界の状況を認知し、それを基に運動を企画・実行するが健常者なら全ての感覚入力に対して自動的に反応するのではなく、何に対してどう反応するか、又は反応しないか等、意図による制御が働いている。補足運動野が損傷されるとこの意図による制御が働かず、感覚入力によって自動的に運動がトリガーされてしまうと考えられる[19]

順序動作の制御

 複数の動作を適切な順序で実行すること(例.カップ麺の蓋を開けて"から"湯を入れるなど)は日常生活の中で重要な役割を持つが、補足運動野の損傷は一連の動作を順序立てて実行することが困難になる運動障害を引き起こす。その一方で個別の要素的運動を実行する限り目立った障害を示さない[20]。健常者に於いても順序動作の実行に伴って補足運動野の脳血流が増加すること[21]、複数の動作を個別に行うよりも一連の動作として行う時に補足運動野上から記録される運動準備電位(Bereitschaftspotential)が増強すること[22]が指摘されている。動物実験でも補足運動野、前補足運動野には動作の順序に選択的な活動を示すニューロンが数多く存在すること、GABA受容体(GABAa)作動薬であるmuscimolをこれらの領域に注入することによって順序動作の実行が障害されるなど人間で得られた知見を支持する結果が得られている[23]

両手の協調運動

 片手で木の枝を引き寄せてもう片手で実を取る、両手で糸を結ぶなど目的を達するために両手を協調させて動作させることは様々な場面で見られるが、補足運動野の傷害は両手の協調動作にも重篤な障害をもたらす。例えばサルにアクリル板に開けた通し穴の中のレーズンを取らせると、片手でレーズンを穴の反対側に押し出しもう一方の手で受け取ることが容易に出来る。ところが補足運動野を損傷したサルでは穴の両側から同時に両手で押し出そうとして取り出すことが出来ない。対照的に運動前野を損傷したサルではこのような両手の協調運動障害は見られない[24]
